It's a print thing.  I saved mine as a PDF and have it stored on my PC.  Of course, I forgot to bring it with me when I picked up our last refill.

Mine was directly from the dr's office. It's an open use 3rd party insurance card on a marketing pamphlet. Unless it's very different from the one I used the only reason to get a name is for shipping information and marketing purposes. I'd use a parent name. HTH.

FYI only because I used it at Sam's Club last month, they had a POS (point of sale) problem processing the co-pay card for more than one twin pack. But we're on the epinephrine family plan so if you're only buying one twinpack at a time it may work normally. In our case the purchase software wanted one co-pay care per twinpack for simultaneous purchase.

And yeah, I would've had the pharmacy run separate sequential transactions but it was DH in there.

Just wanted to post in case people weren't aware of this, as I was not.  If you go to www.epipen.com and click on the "start saving now with your epipen copay card" link, you can get a card that will pay your co-pays for up to 3- 2pack refills through 12/31/13.  Maximum benefit is $100.   Getting mine now!
